About ExSEPSIS ExPRESS
The HAP Hospital Improvement Innovation Network (HIIN) has developed the ExSEPSIS ExPRESS program to provide hospitals with real-time data tools to help identify and monitor sepsis survivors post-discharge. In partnership with PatientPing, the nation’s leading care coordination company, hospitals participating in ExSEPSIS ExPRESS will have access to a custom dashboard. ExSEPSIS ExPRESS allows health care providers to identify sepsis survivors post-discharge. Through the PatientPing “Stories” feature, hospitals will be able to follow sepsis survivors through their post-discharge care transitions to better understand readmissions trends.
About PatientPing
PatientPing continues to grow a national network of thousands of hospitals, post-acute facilities, health plans, HIEs (Health Information Exchanges), and care providers throughout the United States. Through the ExSEPSIS ExPRESS program, PatientPing provides hospitals participating in ExSEPSIS with a real-time dashboard and Stories platform to:
- Identify sepsis survivors following initial hospital discharge and throughout their post-acute care transitions
- Monitor sepsis survivors’ care events to provide timely interventions and proper care delivery
- Support readmission reduction goal following a sepsis discharge to improve patient outcomes and avoid costly penalties
Getting Started with Ex SEPSIS ExPRESS
The HAP HIIN strongly encourages all hospitals participating in the ExSEPSIS initiative to join the ExSEPSIS ExPRESS program to improve care coordination for sepsis survivors. There is no cost to join the program. Please follow the steps below to get started:
