About ExSEPSIS ExPRESS

The HAP Hospital Improvement Innovation Network (HIIN) has developed the ExSEPSIS ExPRESS program to provide hospitals with real-time data tools to help identify and monitor sepsis survivors post-discharge. In partnership with PatientPing, the nation’s leading care coordination company, hospitals participating in ExSEPSIS ExPRESS will have access to a custom dashboard. ExSEPSIS ExPRESS allows health care providers to identify sepsis survivors post-discharge. Through the PatientPing “Stories” feature, hospitals will be able to follow sepsis survivors through their post-discharge care transitions to better understand readmissions trends.
 

About PatientPing

PatientPing continues to grow a national network of thousands of hospitals, post-acute facilities, health plans, HIEs (Health Information Exchanges), and care providers throughout the United States. Through the ExSEPSIS ExPRESS program, PatientPing provides hospitals participating in ExSEPSIS with a real-time dashboard and Stories platform to:

 
  • Identify sepsis survivors following initial hospital discharge and throughout their post-acute care transitions
  • Monitor sepsis survivors’ care events to provide timely interventions and proper care delivery
  • Support readmission reduction goal following a sepsis discharge to improve patient outcomes and avoid costly penalties
 

Getting Started with Ex SEPSIS ExPRESS

The HAP HIIN strongly encourages all hospitals participating in the ExSEPSIS initiative to join the ExSEPSIS ExPRESS program to improve care coordination for sepsis survivors. There is no cost to join the program. Please follow the steps below to get started:

1. Reach out to Maggie Mille, sepsis project manager, HAP HIIN at mmiller@haponline.org, Beth Murray, readmission project manager, HAP HIIN at bmurray@haponline.org, or  Zach Mitchell at PatientPing zmitchell@patientping.com for further details on the program
2. Execute participation agreement & business associate agreement

Resources available to ExSEPSIS ExPRESS Participants

  1. ExSEPSIS ExPRESS Participant Readmissions Dashboard
    HAP HIIN has worked with PatientPing to design a sepsis readmission dashboard. This resource is available at no cost to all Ex ExPRESS participants. The dashboard uses participant ADT data, and PatientPing’s national network, to highlight trends in sepsis survivor demographics, post-discharge utilization patterns (including SNF & HHA), and readmission trends.

  1. PatientPing Stories Platform for ExSEPSIS ExPRESS


Participants will have 12 months of no-cost access to PatientPing’s Stories Platform to identify sepsis-survivors in their hospital settings and monitor post-discharge activity and readmissions across the network.

  • Identify sepsis survivors following initial hospital discharge and throughout their post-acute care transitions
  • Monitor sepsis survivors’ care events to provide timely interventions and proper care delivery
  • Support readmission reduction goal following a sepsis discharge to improve patient outcomes and avoid costly penalties
  • Receive customized alerts, emails, and/or SMS on patient events
  • Access PatientPing’s secure, web-based platform via computer, smartphone or through integration with your EHR
ADT IMPLEMENTATION

Easy, Lightweight HL7 Integration

PatientPing requires all hospitals on our network to send unfiltered ADT feeds that include all message
types and segments. This reduces information gaps which can cause serious issues with the delivery of our
service. Our users rely on PatientPing for real-time admission, discharge and transfer data and unfiltered
feeds are a critical part of the care coordination process. We comply with all local and national HIPAA laws
in sharing sensitive data.
